Form used to enter a horse in a PFHA sponsored show. One horse per entry form.

This form is for Senior Certified Stewards to complete after evaluation a Learner Steward.

This form is for judges who wish to judge at The Grand National Show

This form is to be filled out by the Show Manager if they wish to have a guest judge at their show. Each application for a Guest Judge card must be made to the Judges and Stewards Committee at least ninety (90) days before the show in which the Guest Judge will officiate.

This report should be the composite of the show committee of a judge during their show

PFHA JUDGES AND STEWARDS AGREEMENT

ADDENDUM TO SHOW VALIDATION CERTIFICATE

The following is a list of documents you will need to return to the PFHA office within 15 days postmarked after the show

PASO FINO HORSE ASSOCIATION STEWARD REFERENCE QUESTIONNAIRE

PFHA STEWARD’S REPORT AND EVALUATION. This is to be filled out by the show Steward.

This report should be the composite of the show committee of a steward during their show

Form to nominate foal into the PFHA Fabulous Futurity Program

Form to request a microchip to be used to identify the horse. (Microchips are required for horses entered into the Futurity and ID must be recorded with PFHA by July 1 of two-year-old year.)

Agreement to participate in the PFHA Stallion Service Auction.

Form used to officially bid for a stallion service in the PFHA Stallion Service Auction.

Enroll in the Pasos for Pleasure Program.

Records the hours rider rode in a non-pointed activities.

Records miles that a horse completed in the Competitive Trail program.

Records miles that a horse completed in the Endurance Trail program.

Records miles that a horse completed in the Pleasure Long Distance Trail program

Application for participation in the Sport Horse Program

Records points horses earn in the Sport Horse Program.

Form used to participate in the Ticket to Ride program.

Application to host a Trail Horse Test
